Not a conventional romantic comedy. What do you expect from Park, and what do you expect from a plot about a psychiatric ward? The characters are much more than a bunch of kookooz running around... they are cute, funny, at a deeper level they show sympathy, compassion, etc just like most of us do, and this aspect of human nature transcends all boundaries, just that this time expressed through a more creative and amusing way.
The movie could've used less blood but it's purely for theatrical.
