Fellatio. (possibly) real sex. and probably the is it real, is it simulated sex in the rape scene at the beginning's what got it banned (in australia). I found irreversible, which was R18+ (again in australia) way more confronting (and that was simulated sex in an extended ie real time rape scene).
regardless, not something you'd wanna watch with your parents.
