This movie is an experiment in shame. It's not meant to be a portrait of reality but rather hyperbole. The protagonist suffers much the same things any fearful young woman might, but in extreme circumstances.
An STD is life threatening and necrotic, a lover is cold and distant, drugs are more effective than usual. The entire film is meant to display how shame can kill. Blame can kill. Disease, sexuality, self harm.
.. these are all things that torment the victims, they should not be blamed for them.
