Actor Vic Morrow and two child extras, My-Ca Dinh Le (7) and Renee Shin-Yi Chen (6), died in July 1982 during the filming of Twilight Zone: The Movie, as a result of an accident with a stunt helicopter.
The tragedy took place during filming of Segment 1, directed by John Landis. A pyrotechnic explosion caused the pilot to lose control of the helicopter. It went down, crushing Chen and its blades beheading Morrow and Le. All three died instantly.
