It really didn't need a narrative focus as one critic put it. It is about the music that silences the voices in a schizophrenic's head. I think that having a "narrative" focus would have detracted from the experience of being schizophrenic because there is nothing linear about the way that someone in the grips of psychosis is aware in the way a non-psychotic person could ever understand.
I think Jaime Foxx does a pretty credible job of playing someone suffering from schizophrenia, and all he has to silence the voices is his music since it is made pretty clear that he is not on medication.
And, Robert Downey Jr. is quite believable as a man who is wrangling with his own mental issues.
