I guess I have a higher opinion of this movie than the critics. Personally, I saw Vic's world as a place inside Vic's schitzophrenic mind, and the main people representing different aspects of his psyche; Ben as his "Brass Balls", Mickey as his wit and reactiveness, etc...
And the whole thing is happening in Vic's head while he is actually still in the Asylum.
