It's incredibly difficult to attempt to review or even describe this film.
Herzog fans need to see it, as do fans of strange cinema. There are some serious thought-provoking undertones to this film, and the symbolic usage of dwarfs as the cast can be interperted as feeling helpless in a reckless world or the general feeling of one's self shrinking from everything around them.
However, you don't need to analyze the film like that to enjoy it. Enjoy it for what it is at its surface -- a movie featuring some little people doing some very strange things.
