All kinds of fucked up. It features people with horrible deformities, gallons and gallons of blood, grotesque violence, weird fetish-y sex and redefines sacrilegious. That said, it's a great movie.
It takes a while to get going and you will be frustrated for the first 20 minutes or so, but by the time it gets to the first gun master, it really starts to shine as something truly otherworldly. I wouldn't recommend it to people easily offended, but to anyone else, it's an absolute must watch.
