The film completely loses its way after its first half and after that, events and motivations seem more focused on just delivering something visually half-interesting than having any meaning or purpose. The first half however, is genuinely interesting, funny/uncomfortable, well-acted, original and with effective use of special effects makeup.
Someone with more than a passing knowledge of medical practice might find things a bit over-simplified, but all-in-all American Mary sticks close enough to reality that it creates a plausible, vivid and intriguing reimagining of the American Dream.
