4 months, 3 weeks and 2 days is brilliance is in it's simplicity. The film mananges to be more powerful, brutal and realistic without falling into melodrama or overstating it's intentions.
The film feels like being a fly on the wall of these two friends lives for 24 hours. A small moment in ordinary people's lives as they try to make major decisions under amazing amounts of stress and supression.
Some will try to argue the "pro-life" vs. "pro-choice" sides of the movie but that's not really the point.
The point is survival. Since the dawning of humankind , people have had to fight to survive and to do that people have had to what they felt is bestregardless of abstract moral and ethical guidelines.
That is the real message behind this film.
