I guess the best way to put it is this: If you like the transgressive fiction of J.G. Ballard or Chuck Palahniuk, then you'll like this movie, but otherwise you might not really care for it much. It's about people on the edge of social acceptability (or a bit beyond) on the edge of sanity (or rather past the edge somewhat), so I can definitely see why some people may not care for it.
And it's definitely not for the casual moviegoer. All that said, if unsettling is your cup of tea, I'd give it a shot. The best analogy I can think of is David Lynch's "Blue Velvet." They're both guaranteed to turn off most people, but you can't shake it once you've seen it if you like it.
