Lifetime-entrap of false hope and depression or feeling fucking blissful and beautiful all around, your call. Violence, greed, fraud, scam, two-faced hypocrism, destructive society is the hit, not heroin.
The movie is like a two faced demon, doesn't matter which side you're on- life or heroin- you're doomed. Trainspotting, counting the trains as to an opportunity to choose life & flee or counting trains to kill monotony and blankness of life through drugs.
It's just how you see it, leaving the movie open ended.
