The storyline is 99% identical to the Japanese version (they had to change it a little bit due to the American actors of course) and quite a bit toned down compared to the Japanese release, even in the unrated version.
But they make up for it with a hell of a lot more of those "gotcha" moments. The first time I saw the scene where Toshio pops up in front of the screen and opens his mouth with that damn cat sound I literally jumped, said "What the f*** was that???" and rewound it to catch it.
