For all those film's horrible failings - and there are plenty - I think it can still be poignant. Way too much cliche, convenient un-reality, and preaching, but a tragic tale is still told. Not the terminally ill boy; these things happen and there's only so much we can do.
The girl, on the other hand, is a victim of what this world can do to people. Thrown into a superficial world, she loses what she was. SPOILER - and I hope at the end he didn't truly get his wish.
Not only is that super-unrealistic, but it would be just one more stage where her beauty is used as a tool.
