One of the most horrifying movies I've ever seen. Genuinely hard to watch, even for someone who's pretty much "seen it all". It's not just mindless violence or torture porn though, but actually has something to say (although it is fairly ambiguous as to what it is).
Also, the performances of the actresses and the general filmmaking skills on display are quite impressive.
