A ventriloquist/magician's fear of failure and success lead him to develop a split personality which attains a voice through his puppet. I saw this as a kid and it put me off ventriloquists - I still find those dummies creepy.
hopkins (in a pre-hannibal role) shows he's got a knack for played mentally disturbed characters.
