Except for the names, it's almost completely unrelated to the book (which I love), and definitely the worse for it. The nihilism and alienation that made the novel so engaging are gone, along with Clay's drug use and nearly all of the gay content.
What's left is Julian's addiction(though his substance of choice is cocaine, not heroin), an inexplicable love triangle between Blaire, Clay and Julian, and a look that's unmistakably eighties.
The result is, excluding the uneasiness of watching Robert Downey Jr. play an addict in the midst of a breakdown, just plain boring.
