I think the purpose of the film is to present the psychological of a person who suffers from certain disorders - as it is presented during the same. It really comes to a point where you believe what the protagonist is going through and at other times you doubt it.

Of course, for her everything is real, everything is in her mind. Perhaps the traumas forced her brain to protect itself against everything bad and created an entirely unique world within itself.
