There's a reason why this one always makes the "best horror films" of all time lists. It features a) one of the most profoundly disturbing scenes in film history b) one of the best most satisfying twist endings in film history.
This is definitely a film that delves into a pathology and makes it terrifying without resorting to exploitative tricks.
