Unlike human centipede II and some of the later saw movies, this is not 100% senseless gore. It is shocking, over the top, horrifying, and it will test the limits of what you are willing to let yourself watch, but it is not nonsensical.
I'll compare it to the human centipede II (THCII), because I think it is the easiest to compare with. Whereas THCII seems to chase violence for violence sake, this film does so to intensify the story, and the connection with the main character, who, unlike the main character in THCII, is relatable.
A serbian film has just a bit more depth to it than does the human centipede. It is not a plotless mess. It is however, very hard to watch.
