Unlike the trend of real serial killer movies (ie BTK, Ed Gein, Gacy, et al) this one is not only a mature, serious work it is also surprisingly (and thankfully) restrained in its approach to the material since Albert Fish was a child molester/cannibal.
But understand this is not a horror movie or a thriller; it's more of a psychological drama about the last few years of this sick individual and the cop who is determined to arrest him.
