Like so many Japanese horror films it takes too long to get interesting. I recommend fast forwarding to the good stuff. The intro and build up is very unnecessary all you need to know is a young mentally unstable girl goes in for an audition for a phony role.
Unfortunately for her new boyfriend she's a sadist who likes using piano wire. Her creepy German doctor getup and paralyzing drugs make the torture scene realistic and reflect her inner serial killer which is typically masked by her beauty and coy appearance.
