The truly horrifying thing about this movie is the portrayal of terrible parenting, and the stupidity of nearly all the main characters. I've never been as annoyed by a protagonist as I was by the obnoxious, apparently mentally deficient little boy.
But then you get to know his parents, and it makes a little more sense. He plays a video game that condones murder and sending people to hell, confuses it with reality, and his parents see nothing wrong with it? Now THAT is horror.
But as far as this being actually scary, not so much. The acting is so bad and the effects so cheesy that it's impossible to actually be creeped out by what's happening on screen. But this is definitely disturbing in that you can't imagine someone not being concerned that their child is so twisted and separated from reality.
This is worth seeing, but it's really more of a social statement than a horror movie.
