For what it is, a fake murderous rampage filmed on a hand held camera, the Special FX are flawless, the atmosphere is dreadful and some of the things they get the actors to do would make Zed's Gimp blush.
Dirty, sickening, horrible but if you are into that thing go for gold.
5 starts FOR WHAT IT IS.
